Formal Complaint Form

The purpose of this policy is to provide a clear, fair, and respectful process for addressing complaints related to coaches, players, umpires, or fans. This policy applies to all participants and attendees involved in Esko Youth Baseball Association activities.

I understand that by filling out this form, you are submitting a formal complaint as outlined in the Esko Youth Baseball Associations policies document. We ask that before filing this complaint officially, you have completed the first three steps in the Complaint Policy Process, if possible. These are: Step 1: Discuss the issue directly with the person involved, if appropriate and safe. Step 2: If unresolved, report the issue to the team coach (if the complaint is not about the coach). Step 3: If the complaint involves a coach or remains unresolved, escalate it to the Level Coordinator. Once this complaint is received, the investigation, resolution and disciplinary action processes will start, if applicable. Do you understand that this is a formal complaint?
